Chest discomfort without pain, almost my entire life?

I have constant chest discomfort, not pain, that’s been ongoing my entire life. Doctors have always thought anxiety, and I’ve been on anxiety meds for quite a while, but the tightness has never gone away. I do not have asthma or any other asthma symptoms. It gets to the point that I want to cry at times. I am a smoker and have been smoking for a good number of years. Someone please point me in the right direction.
