How do I ask for help for social anxiety when the problem is a fear of people?

I’ve been experiencing social phobia for many years and although I have come a long way (i no longer seize up with fear in public places and can now make phone calls like ordering home delivery), my life is still extremely held back by my fears. I so want to see a specialist and ask for help and i often daydream about having the confidence to explain my situation and ask for help but when it comes down to it i just cant go through with making an appointment. I imagine horrible situations where i cry uncontrollably and clam up and am unable to even talk (a very likely possibility) and the specialist getting angry with me. How can i get help when im too terrified to ask?
